Overview
We are seeking a motivated and relationship-driven Hospice Community Liaison to support the growth of our hospice services throughout San Jose. This role focuses on building strong partnerships with healthcare providers and community organizations to increase awareness and referrals.
Key Responsibilities
- Build and maintain strong referral relationships with hospitals, skilled nursing facilities (SNFs), assisted living facilities (ALFs), physicians, and other healthcare providers
- Promote home health and hospice services through regular in-person visits and outreach efforts
- Identify new business opportunities and help expand service areas
- Coordinate closely with clinical teams to ensure smooth and timely patient transitions
- Track referrals and maintain accurate marketing activity logs and reports
- Represent the agency professionally within the healthcare community and at events
- Maintain a consistent presence with referral sources to strengthen long-term partnerships
Qualifications
- Experience in healthcare marketing, home health, or hospice preferred
- Bachelor's degree in health care or equivalent, marketing/sales or public relations, preferred.
- Strong communication, networking, and relationship-building skills
- Knowledge of Medicare, insurance guidelines, and referral processes is a plus
- Self-motivated with the ability to work independently and meet growth goals
- Reliable transportation required for field-based visits
